Office Hours
Office hours are 8:30-5:00 Monday through Friday. The
office is closed from 12:00-1:00 p.m. for lunch.
The office is closed for most major
holidays.

Emergencies
For emergencies after hours call 911 or go to the
nearest emergency room. Urgent
requests after hours will be forwarded to the answering service
who will relay the message to the physician. |
Telephone Policy
It is not our policy to treat illness
over the telephone.
If you are ill, make an appointment so we may see you
in the office.
We do not prescribe antibiotics or any controlled
substances (pain or nerve medications) without first examining
the patient.
We will do our best to handle minor problems over the
telephone.
You will be asked to leave your phone number and a
message.
Your call will be routed to the appropriate staff to
answer your question.
Every attempt will be made to return your call
promptly.
Our phones are answered from 8:30 a.m. to 4:30 p.m. on
weekdays. Calls not within this time frame will be automatically
forwarded to the answering service, which will alert the office
or physician.
Non urgent messages received after 4:00 p.m. may not
be returned until the next business day. |
Appointments
Patients are seen by appointment only.
If, however, you have an acute problem that needs
immediate care, we will make every effort to work you in between
scheduled patients.
Scheduled patients will have priority.
Please call beforehand so we may check the schedule
for the most convenient time, minimizing your wait.
If you are more than 10
minutes late for a scheduled appointment, we may ask you to
reschedule.
If you cannot keep your appointment we ask that you
give 24 hour notice of cancellation.
No show appointments may be subject to a $25.00 fee.

Office Visits
On your first visit to the office, please arrive 30 minutes
early so that we may obtain information essential to your care.
To comply with identity
theft laws, we require two forms of identification at your first
office visit, one being a picture ID.
Please be assured that all information is confidential
and none of your records will be released without your written
permission.
If you would like for us to obtain your previous medical
records from another office or physician, medical record release
forms are available from our receptionist or on our website.
As you leave the office, please stop by the checkout window
to obtain current charges and schedule your next appointment.
You will receive an appointment reminder with date and
time. |
Fees
Office visits are payable at the time of service.
We accept assignment with most insurance plans
including Medicare, BCBS, Cigna, Aetna, Humana, and United
Healthcare.
Deductibles and co-pays are due at time of service.
For your convenience we accept MasterCard and Visa.
Your insurance policy is a contract
between you and your insurance company.
Responsibility for payment of the bill remains with
you, the patient.
We will file primary and secondary insurance claims
for you.
A statement of any unpaid balance will be mailed and
will be due within 10 days of receipt.

Prescription Refills
We prefer to refill all prescriptions during routine office
visits so we are able to maintain accurate records of your
medications.
Please try to take care of this matter before you run
out of your medication.
Since prescription refills need to be noted in your
medical record, no routine prescriptions should be handled after
regular office hours or on a weekend.
If you have been seen recently but forgot to have your
prescription refilled, please call during regular office hours.
Many patients have prescription drug plans that
require form completion or preauthorization.
We take care of the majority of these issues one day a
week. Please give us ample time before you are out of your
prescribed medication if form completion or preauthorization is
necessary for your prescription drug plan.

Hospitalizations
Dr. Parish admits patients to Lourdes and
Western Baptist Hospital.
If you need to be hospitalized, we will make the
necessary arrangements.
Charges for hospital visits are completely separate from charges
generated by the hospital, radiology or laboratory department. |
Call Group
Dr. Parish shares call with Dr. Kenneth
Cook, Dr. Aribee Martin, Dr. James Eickholz and Dr. Kyle Turnbo.
